It sounds like your Shih Tzu is experiencing quite a bit of discomfort, possibly due to skin irritation or an allergic reaction. Let's go through a few steps that might help alleviate the situation.

First, even though you're using chewables or pills for flea and tick prevention, it's a good idea to check your dog's fur and skin for any signs of these pests. Sometimes, just one bite can trigger a reaction.

Since coconut oil hasn't been effective, you might want to try an oatmeal-based shampoo or a hypoallergenic shampoo specifically designed for dogs. These can be quite soothing for irritated skin. Just be sure to rinse thoroughly to avoid leaving any residue that might further irritate the skin.

To prevent your dog from causing more damage by scratching, you might consider using an Elizabethan collar, or cone, temporarily. This can help the skin heal without further irritation.

Given that your dog has scratched to the point of bleeding, it's important to monitor the area for any signs of infection, such as redness, swelling, or pus. If you notice any of these signs, it would be best to consult a veterinarian.

Also, think about whether there have been any recent changes in diet or environment that could have triggered an allergic reaction. Food allergies or environmental factors like pollen can sometimes cause itching.

If the scratching continues or worsens, or if you notice any signs of infection, it would be wise to consult a veterinarian for a thorough examination and appropriate treatment. Please feel free to reach out if you have any further questions. Thank you.
