Thank you for reaching out to us. Did he encounter any unusual or unexpected triggers or events outside before? First of all, you may want to make the door a great place. You can place a treat or praise him whenever he is close to the door or ready to go out. You can also consider a portable crate to transport him to outside and gradually lure him out with treats. Make sure you use a high-value treat. Calming aids such as calming cap may reduce his fear or anxiety outside as well.
Whether it is at the door or outside on the grass, the purpose of training is desensitization. The procedure is much clearer online especially on YouTube as it is very hard to explain in words. However, the basic principle of desensitization is to compensate for a particular behavior of a dog that is usually stressed out. In other words, it is a sort of counter-condition. In this case, it is compensating him for going out to walk(what he dislikes at the moment). You may want to use a "clicker" for this method and the use of a "clicker' can be easily learned on Youtube as well. However, if it is showing no improvement, you do have to consult with either a dog train for a non-medical method or your vet regarding the prescription of antidepressants and relevant psychotropic drugs.
