It sounds like Bella is experiencing some discomfort with her ears, which is causing her to shake her head and pant. Given her age and existing conditions, it's important to ensure she is as comfortable as possible.

Head shaking in dogs is often a sign of ear issues such as infections, mites, or irritation. Since Bella is already on an ear cleanser, it’s good to continue using it as directed. Here are a few steps you can take to help her at home:

First, make sure you are cleaning her ears gently and as recommended by your vet. Over-cleaning can sometimes cause irritation, so it's important to follow the prescribed frequency.

Next, look inside her ears for any signs of redness, swelling, discharge, or foul odor. These could indicate an infection or other issues that might need more than just cleaning.

Given that Bella has sundowners syndrome, maintaining a calm and quiet environment can help reduce stress, which might be contributing to her panting.

Additionally, keep an eye on her for any additional symptoms like ear scratching, loss of balance, or changes in behavior. These could indicate a more serious issue that might need veterinary attention.

Ensure she has access to fresh water and a comfortable resting place. Panting can sometimes be a sign of pain or anxiety, so making her environment as stress-free as possible is key.

If her symptoms persist or worsen, or if you notice any signs of severe discomfort or infection, it would be best to consult your veterinarian. They might need to examine her ears more closely and possibly prescribe medication to address any underlying issues.

If you have any further questions, please feel free to ask. Thank you.
